Who this policy covers
This policy applies to this website and the consultation, Medicare Scope of Appointment, and Marketplace consent forms linked from it. Insurance services are provided by Vincent Guglielmo or another appropriately licensed insurance producer acting in the producer’s individual licensed capacity. Artemis Agent Services LLC provides administrative support.
Information collected
The general consultation form may collect contact information, location, preferred contact method, and a general description of the requested help. Separate forms may collect information needed to document a Medicare Scope of Appointment or Marketplace consent and application-review confirmation.
Do not send health information, medical records, Medicare beneficiary identifiers, Medicare numbers, Social Security numbers, payment-card information, bank details, tax returns, or account passwords through the general form or general inquiry email. If additional records are needed, a separate method and purpose will be explained first.
How information is used
Information is used to respond to a request, document permission to make contact, schedule a consultation, provide requested administrative or insurance assistance, keep required compliance records, and protect the security and integrity of the service.
Submitting a form permits a response through the contact method selected. It does not authorize enrollment, guarantee eligibility or coverage, or require a purchase. A referral from another person does not authorize contact. The referred person must initiate the request.
Service providers and disclosure
Website hosting, form hosting, communications, and other service providers may process information only as needed to provide their services. Information may also be disclosed when required by law, regulation, a valid legal process, or to protect rights and security. Personal information is not sold.
Retention and security
Records are retained only as reasonably needed for the request, business operations, applicable insurance or Marketplace requirements, dispute resolution, and legal obligations. No internet transmission or storage system can be guaranteed completely secure.
Your choices
You may ask to correct contact information or withdraw permission for future contact by emailing vinnytheinsuranceguy@protonmail.com. Withdrawal does not affect actions already taken or records that must be retained. Message and data rates may apply to texts.
